Hall of Fame Nomination Form-United States Flag & Touch Football (USFTL)

Minimum of 10 years participation at the National Championship Tournament lever as a player, coach, sponsor, referee, director or supporter
Players Only: (5) 1st Team All American selections at "Super/A" division only, or
(3) 1st team & (2) 2nd Team All American selections at "Super/A" division only, or
(2) 1st team & (3) 2nd Team All American selections at "Super/A" division only, or
(1) 1st team & (4) 2nd or 3rd Team All American selections at "Super/A" division only
Players playing 1971-1989: (3) 1st Team All American selections
On & off the fields, individuals must respect the games of flag & touch football.
